Paciente de 42 años, sin antecedentes personales a destacar, que acude a
consulta para VALORACION de dolor y limitacion funcional a nivel del pie
derecho, luego de traumatismo durante un concierto
EXPLORACION FISICA
No deformidad. No hematoma. Leve tumefaccion en region dorsal del pie derecho
con dolor intenso a la palpacion a nivel de la articulacion
astragalo-escafoidea. Tobillo estable. Movilidad conservada. Movilidad dedos
conservada no dolorosa. Fuerza conservada. No alteraciones de la sensibilidad.

PARACLINICA
Tomografia Axial Computarizada de tobillo derecho: Fracture compleja de
escafoides tarsiano derecho, sin desplazamiento, impactacion ni angulacion de
los fragmentos, ni ablacion del tuberculo interno
JUICIO DIAGNOSTICO:
Fractura de escafoides tarsiano derecho
Tratamiento
1. En agudo: pierna elevada, antiflamatorios no esteroideos por boca y bolsa de
hielo
2. Caminara con ayuda de bastones
3. Carga parcial progresiva
4 Ibuprofeno 600 mg v/o cada 8 horas hasta remision sintomatica
5 Heparina IBOR 3500 Unidades subcutanea cada 24 horas hasta comenzar apoyo
6 Fisioterapia tutelada en centro especializado
7 Vigilancia evolutiva y revision en 6−8 semanas

with severe palpation pain in the joint
astragalus-scaphoid. Stable ankle. Mobility preserved. Mobility fingers
preserved not painful. Strength conserved. No sensitivity changes.
Distal pulses present
PARACLINIC
Axial computed tomography of the right ankle: complex fracture of
right tarsal scaphoid, without displacement, impact or angle of
fragments, or ablation of the inner tubercle
DIAGNOSTIC JUDGMENT:
Right tarsal scaphoid fracture
Treatment
1. In acute: elevated leg, nonsteroidal anti-inflammatory by mouth and bag of
ice
2. He will walk with the help of sticks
3. Progressive partial load
4 ibuprofen 600 mg v / o every 8 hours until symptomatic remission
5 heparin IBOR 3500 units subcutaneous every 24 hours until starting support
6 physiotherapy supervised in specialized center
7 evolutionary surveillance and review in 6-8 weeks
